How to attach fbx items...?

The long and short of it. There's some cosmetic items that can be attached to a VRC avatar/Unity model, and while I can generally equip items and head cosmetics - I can't seem to get items to stick to the model in Unity, despite it being listed as a unity accessory.
IE: the best I can do for some cross-tape pasties is to armature link them to the chest, and they don't follow what they're supposed to be attached to! They just kind of float at chest level and turn, but aren't 'connected' to the physics and model below. Am I missing something, or is this a Blender issue that should be handled before even going to Unity?
