ive been having a similar issue, i think the hat tool isnt working, the head wont attach and the dynamics wont work

The avatar I have started having the head sink into the chest area instead of the head for my end. I look fine to other people, but to me it's very jarring. I know this happened because of a VRCHAT update.

From the avatar website:
