Here is a prefab:
You will need both the avatars in your Unity project, along with Modular Avatar and VRCFury.
Feel free to ask if you need any more help. -
If you're only working in Unity and you have both avatars' assets in your project, there's no major reason aside from optimizing your avatar to have a combined fbx, just drag and drop the prefab into your scene and it will work fine. If you're asking for a combined fbx to use in Blender or for some other reason that isn't dressing up and customizing an avatar to upload it (which the prefab will allow you to accomplish), that goes beyond what I'm willing or able to do as a volunteered service.
