Mainly just talking about creators like Sinensian or Denni with their thick Imeris and Kikyo avis. I'd assume it's a lot of custom blender work or a comical amount of blendshape abuse

It's very easy to do in pose mode in blender as well. Much more reliable.

decided to give the tutorial a try. here are the results vs how i usually thicken models in pmx editor
Blender version on the left, pmx-editor version on the right
                  alt text

not too much of a difference, was able to make the boobs rounder in blender and the arms chunkier too.
You have a bit more control in blender but it takes a bit longer. and require more steps. Admittedly i didn't finish the tutorial properly and thus had an extra body mesh i had to delete and the head had no armature for some reason and thus doesn't work with animation in mmd.
not sure it's worth it if you don't already know you're way around blender.
And it's probably dependant on the base body you start with. I'm guessing a taller body is better (and why they are using an imeris base with headswaps) otherwise the model starts looking too fridge like

Blender is the best way. Bone scaling usually doesn't come out properly or in a way that looks normal/accurate, So sculpting is absolutely the way to go.
