pictures of the errors or setup would help

1. find and highlight your FBX (not just the prefab).
2. tick the "read/write" and "legacy blendshape normals" boxes
3. hit apply

this is usually caused by not having physbones from the outfit set correctly. make sure the breast bones that are / were part of the outfit are in the "ignore transforms" section of the physbones component, or don't have their own physbone components.

if you have a base selestia model, just add one thing at a time till it breaks, then try to fix that part

i would suggest using Modular Avatar so you'll never have to worry about permanently breaking anything.

VRCFury has a component called "add full controller", it will mix this one into the existing one when uploading or entering play mode (so it can't ruin what you already have)

i assume you mean "place in the world, and keep it from moving after that"? VRCFury has a "Toggle" component you can add to the prefab. Make the Toggle be "World Drop". Then have the dildo be attached to your hand, hold it where you like, and activate the toggle. Boom, it'll stay where you dropped it. There IS also the issue of physbones in the dildo going nuts while you move around, the way to fix this is have a movement restraint like the one found in GogoLoco. This would also let you line up the VRC dildo with one IRL without having to worry about accidently nudging the controls and fucking the alignment up.
